The following documents are needed to register your son or daughter:
VERIFICATION of ADDRESS:
Present one or more of the following items in the parent/guardian's name (see Residency Verification Form below the right panel):

Current utility bill: electric, water or Southern California Gas Co. (complete form). School may accept a Letter of Residency from Southern California Edison.
Current property tax or income tax documents (from the IRS, State, and/or County)
Escrow papers with closing date (within 90 calendar days of student entering school)
Rental/lease agreement or payment receipts and, if applicable, Rental Supplemental Form
Current payroll stub, unemployment check or evidence of disability benefits (both name and address must appear on the checks)
Current Social Services placement documents
Correspondence from a government agency.

It is the responsibility of the parent/guardian to obtain this information from the previous school at the time of checkout.

Records may be obtained from previous school / NO entry if immunizations are not current.
PROOF OF BIRTHDATE
Acceptable proof includes any of the following: birth certificate or passport only.
CERTIFICATE OF PROMOTION
Ninth grade students new to the PYLUSD must provide a Certificate or documentation promoting them into the ninth grade.
This form is required when the student is living with someone other than the Parent/Legal guardian and must be obtained from our Child Welfare & Attendance office.
This form will be given to you from El Dorado, Esperanza, or Yorba Linda High School.
SPECIAL EDUCATION FORMS
Current IEP and psych report must be provided before enrollment. This information should be obtained from the previous school at the time of checkout.
Current 504 plan must be provided before enrollment. This information should be obtained from the previous school at the time of checkout.
ASSESSMENT TEST RESULTS
These results must be given to you at the Assessment Center next to Topaz Elementary School.
IF YOU ARE LIVING WITH OTHERS
A notarized "Co-Resident Supplemental Form" that is attached to the two proofs of verification of address that is dated within the last 60 days or bring in the person whose name appears on the two proofs of address to our office to complete the "Co-Resident Supplemental Form."
The registration packet must be COMPLETELY FILLED OUT and must be legible. Be sure to include phone numbers. Emergency phone numbers are very important! Please note who is to pick up your student in case of an emergency. Keep your papers together and return at your appointment time already filled out. If you don't have any of the documents filled out, or if you come in 10 minutes late, your appointment will be rescheduled.
